1. Assumption of Risk
I understand and acknowledge that baseball is a sport involving inherent risks, including but not limited to: being hit by a ball or bat, collisions with other players, slipping or tripping, weather-related hazards, and other conditions that may cause injury. I voluntarily choose to participate in the All Play Baseball Inc. Adult League with full knowledge of these risks and agree to accept all responsibility for any injury, illness, or other damages I may sustain.
2. Release of Liability
In consideration for being allowed to participate, I, for myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless All Play Baseball Inc., its directors, officers, employees, volunteers, agents, and sponsors, as well as any owners or operators of the fields or facilities used for league activities, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss that I may sustain, whether caused by the negligence of the released parties or otherwise.
3. Medical Authorization
In the event of an injury or medical emergency, I authorize All Play Baseball Inc. representatives to secure necessary medical treatment on my behalf. I assume full responsibility for any and all medical expenses incurred.
4. Rules and Conduct
I agree to abide by all league rules, regulations, and codes of conduct. I understand that failure to do so may result in suspension or removal from league activities without refund.
5. Acknowledgment of Understanding
I have read this waiver in its entirety, fully understand its terms, and register voluntarily. I understand that by registering, I am giving up substantial legal rights, including the right to sue.
